TERMS AND CONDITIONS

ALLOWING PERMISSION

I give Mallory Chase and Kevin Chase (hereinafter referred to as “members of MK Quantum Healing”) permission to connect with my subconscious to obtain information regarding my concerns during assessments or sessions.

I also give members of MK Quantum Healing permission to connect with my subconscious to obtain information regarding my concerns should I personally request additional information outside of a session relating to a work done during an assessment or session. Members of MK Quantum Healing will not access my subconscious for any other reason or without my permission.

 

INFORMED CONSENT

1. I understand that The Emotion Code, The Body Code, and The Belief Code System, as taught by Dr. Bradley Nelson (hereinafter called “these methods”), and as practiced by members of MK Quantum Healing, seek to identify and eliminate underlying imbalances by releasing energetic imbalances in the areas of emotions, energy, circuitry, pathogens, structure, toxicity, nutrition, and faulty or outdated beliefs. These methods of energy healing aim to  promote harmony and balance within helping to support the body's natural ability to heal. Energy healing such as these methods is widely recognized as a valuable and effective complement to conventional medical care.

2. I agree to inform my practitioner if I have any magnet-triggered devices (such as pacemakers, hearing aids, certain types of stents, etc.). These devices are contraindicated for release on the physical body using magnets. If such a device is present, the work may still be done using alternative methods, such as without magnets or through proxy/surrogate work.

3. I understand that releasing trapped emotions or correcting energetic imbalances using these methods, as practiced by members of MK Quantum Healing, is not a substitute for medical care. This information is not intended as medical advice and should not be used for medical diagnosis or treatment. Information received is not intended to create a physician–patient relationship, nor should it be considered a replacement for consultation with a licensed healthcare provider. It is not meant to replace any medical treatments prescribed by physicians or other healthcare professionals.

4. I understand that if my practitioner makes suggestions regarding supplementation (including vitamins, minerals, herbal preparations, compounds, or other remedies), I choose to use or ingest such remedies at my own risk. I understand it is recommended that I seek the advice of a physician before using any suggested remedy.

5. I understand that my practitioner will not attempt to change or release any belief system that I still consciously choose to keep. The use of The Belief Code is purely complementary and intended to help release outdated belief systems that I acknowledge no longer serve me.

6. I understand that in approximately 20% of sessions, the release of trapped emotions or other energies may result in “processing,” where echoes of released energies may manifest as temporary emotional or physical discomfort. I understand that this processing may be a normal part of regaining energetic balance.

7. I understand that my practitioner makes no claims regarding healing, recovery, or prevention of any illness and makes no guarantees regarding outcomes. I understand that my use of any information received is at my own risk.

8. I understand that if I have health concerns, I am advised to seek guidance from an appropriate medical professional before making decisions about my health. These services are offered as complementary support and are not meant to replace medical treatment.

9. I understand that sessions are confidential. Personal identifying information (PII), including name, date of birth, and session notes, may be shared between members of MK Quantum Healing for continuity of care if I choose to work with more than one practitioner. My information may also be used anonymously (with all identifying information removed) for educational or research purposes, subject to applicable South Dakota and federal laws.

10. I understand that I am encouraged to educate myself about this work by visiting Dr. Bradley Nelson’s website at https://discoverhealing.com and/or by reading his books The Emotion Code and The Body Code.

11. I understand that MK Quantum Healing is an independent entity and that Discover Healing, Dr. Bradley Nelson, and their faculty or staff are not responsible for the actions or statements of MK Quantum Healing.

12. By agreeing to these terms and conditions, I fully consent to participating in Emotion Code, Body Code, and/or Belief Code sessions with members of MK Quantum Healing (Mallory Chase and/or Kevin Chase).

 

CANCELLATION POLICY

We require 24 hours’ advance notice for all cancellations.

Missed appointments, no-shows, or cancellations made with less than 24 hours’ notice will not be refunded or credited.

If canceling a session from a package with less than 24 hours’ notice, that session will be deducted from the package.

If we are unable to reach you at the scheduled time of your appointment and do not receive a response within a reasonable timeframe during the scheduled session window, the appointment will be considered a no-show and will not be refunded or credited.

 

REFUND POLICY

Refunds will not be issued for missed appointments, no-shows, or cancellations made with less than 24 hours’ notice.

Refund requests for services that have not yet been completed must be submitted more than 24 hours prior to the scheduled session time. Approved refunds will be processed for the amount paid minus any payment processing fees incurred at the time of purchase, as these fees are non-refundable to MK Quantum Healing.

Once a session has been completed, no refunds will be issued.

For session packages, unused sessions may only be eligible for refund if no sessions within the package have been used. Once any session within a package has been completed, the remaining sessions are non-refundable.

By purchasing a session, package, or service with MK Quantum Healing, you acknowledge and agree to these refund terms. You also agree to contact MK Quantum Healing directly to resolve any concerns before initiating a dispute or chargeback with your financial institution.

 

CLIENT ACKNOWLEDGMENT

By scheduling and participating in a session with MK Quantum Healing, I acknowledge that I have read, understood, and agree to these Terms and Conditions.
